I remember being a little perplexed by this at first too. I think the more that I just got into the game though, the more I was just willing to go with the flow and accept things for what they were and not question, just because I usually got distracted with something else.

You can either play on a public server, create and host a server and play on it, or play on lan. For casual gaming, the best method would be to rent a minecraft server from server.pro for free and play on that.
Linking up with someone on the save server is the only way that I have ever done it, but it sounds like there are some other options out there, which is always good.
Yeah, you can create a server for your own map and give the IP to your friend. You can even do this on a map that you've been playing for a while. You need a good internet connection though, otherwise the server won't run at all.
As others have said, a two player game on Minecraft isn't as simple as just plugging in another controller because you are both playing in the same (but different) worlds so you are going to need two systems linked together. LaN is the easiest (and most popular) way to do that I would have thought but again, you are going to have to play on the same network environment rather than it being just as simple as a lot of other plug in and play video games that are around these days.
Just get a server.pro server and your good to go, sometimes you face difficulties with Lan as well, and you cant add plugins and many other features. Server.pro is bae.
I never had that kind of curiosity (and I still don't have it ) because I never imagined myself playing with another person in the same Minecraft world. I prefer to play it alone.
It is because of the server. You can see that more than two people can play using the server they are got themselves into.
If I had a partner to play with, I will gladly apply the methods given here so I could play with the person in the same Minecraft world. But currently, I prefer playing alone and I think it is because I am used to it.
